Sustainability and transformation plans

NHS commissioners and providers have been asked to join together to develop long-term plans for improving health services and outcomes in their area.

From 2016/17 onwards, every health and care system will work together to produce a five-year Sustainability and Transformation Plan (STP), showing how local services will evolve and become sustainable. STPs will be place-based, long-term plans built around the needs of local populations

To do this, local health and care systems will come together in 44 groups or ‘footprints’ across the country. They are currently in the process of developing their STPs for approval by the end of June 2016.
